This workshop will help bridge the gap between hospitals and long-term care facilities. During this workshop, facilities will identify how working together will improve transitions of care. Topics discussed will include social determinants of health to reduce readmissions, conducting root cause analysis through utilization of tools and strategies, nurse to nurse handoffs, advance directives and medication reconciliation.
